Lewis Balyckyi Trust Fund: Application for Sponsorship 2012
The closing date for all applications to the Lewis Balyckyi Trust Fund is midday of the 30/11/2011. We've received the following information for riders interested in making an application. There's a downloadable application form at the bottom of the info.

We set up the trust in Lewis memory as a lasting tribute to a really special person. He was special to many people in many different ways...He always lived life to the full and was always there for his many friends. The Trust has been set up to enable us to raise funds to help a junior/youth cyclist live their dream.
All forms should be returned to lewisbtrustfund@gmail.com, an acknowledgement will be sent to confirm receipt of application.All applications will be assessed by the committee of the Lewis Balyckyi Trust Fund and a shortlist of applicants will be required to attend a final interview held in Lancashire.The application should only be filled out by the applicant and be completed honestly and truthfully to the best of your knowledge.
Basic Criteria
The basic criteria for rider selection are as follows: Must be between 16-18 years of age on the 1st January 2012; Have raced competitively for a number of years; Have a positive attitude and strong desire for a successful cycling career; Agree to attend a CRY screening session (cardiac risk in the young)
After selection
The following will be expected from the successful applicant:
To be an ambassador for the Lewis Balyckyi Trust Fund and behave accordingly at races and public events
If tested positive for banned substances under UCI and BC rules, the sponsorship will be immediately withdrawn and all money to be repaid to the Fund.
To inform the fund if their team or financial situation changesTo send the fund a monthly blog of progress reports and receipts to document the use of sponsorship money
To attend a cry screening session, to be arranged by the LBTF.
